It is a legally binding statement from you saying that you are
assigning the copyright of any code you contribute over to the FSF.
(It also states that you have the right to make such a transfer, and
that you are the copyright holder of the code, prior to its transfer
to the FSF).  If you are employed and the code has been developed for
your employer (or on their machines) then they need to sign a
copyright assignment statement allowing you to contribute their code
to the FSF.  See also:

Fill out a form.  I have attached a copy at the end of this email.

That is good.  Sometimes it takes a bit of time to process the
paperwork, but it is essential in order to make sure that Free
Software remains Free.

The format is very straightforward.  Just look at the entries that are
already in there.  Are you using emacs ?  (Of course you are, all
programmers use emacs. [Ducks to avoid flame war :-) ]).  If so emacs
has a ChangeLog entry creating mode.  Try loading the file you edited
and using C-x-4.
