tc-i386.h tidy


Just moves things around.  Some defines had been added outside the
#ifndef TC_I386/#endif, and other bits and pieces belong inside
#ifndef BFD_ASSEMBLER.  The !BFD_ASSEMBLER stuff can all go, but
I'm leaving it for the time being.
