[gold patch] GDB index 2/2: Add --gdb-index option

This patch adds the --gdb-index option, which builds a .gdb_index
section from information found in .debug_info and .debug_types
sections. If the compiler emits reliable .debug_pubnames and
.debug_pubtypes sections (for which we have a GCC patch ready), gold
will use that information rather than extracting info out of
.debug_info and .debug_types.

Tested on x86_64. OK?
