fork_helper: child died

Often, perhaps more often than not, when I attempt to pipe the output
from one command into another, NT displays the following :Application
Error" window...
The application failed to initialize properly (0xc0000043). Click on OK
to terminate the application.
The application referred to is the application reading the pipe.
In the shell window, the following message is displayed...
fork_helper: child died before initialization with win32 error 128
The command sometimes completes successfully, anyway.
Can anyone suggest what is causing this?
