Thanks for the replies which explain what's going on.
If I leave the PTMX define in the compile fails on
if (ioctl(*ttyfd, I_PUSH, "ptem") < 0)
sockerror("ioctl I_PUSH ptem");
if (ioctl(*ttyfd, I_PUSH, "ldterm") < 0)
sockerror("ioctl I_PUSH ldterm");
if (ioctl(*ttyfd, I_PUSH, "ttcompat") < 0)
sockerror("ioctl I_PUSH ttcompat");
I_PUSH is not defined, on Linux this is in
/usr/include/bits/stropts.h
