It's not clear from the above -- did you redirect the
output of 'mkpasswd -l' to /etc/passwd?  That is,

   $ mkpasswd -l > /etc/passwd

...

Again, did you redirect this to /etc/passwd?

   $ mkpasswd -d -u esurjawinata >> /etc/passwd

...
