The use of <graphic> isn't recommended.
It's being replaced by
<mediaobject>
<imageobject>
<imagedata fileref='file.gif' />
</imageobject>
</mediaobjetc>
You can get an alt text with:
<mediaobject>
<imageobject>
<imagedata fileref='file.gif' />
</imageobject>
<textobject>
<phrase>alt text</phrase>
</textobject>
</mediaobjetc>
