Saxon bug?

Hi,
can anybody explain me what's wrong? I'm writing XSL style sheet
for DocBook DTD document. Output files should be XHTML.
The question is why I always get line like:
<span xmlns:.0="" .0:class="emphasis"><em>something</em></span>
when the source is <emphasis>something</emphasis>.
The output file is not well-formed because of .0, the same case
come on with element <xref>.
I was trying various vrsions of SAXON, but it's same.
Is only way rewriting templates in my XSL, or I haven't something
set?
