I don't know the magic myself.
I use the following customization:

  <xsl:param name="use.extensions" select="'1'"/>
  <xsl:param name="saxon.extensions" select="'1'"/>
  <xsl:param name="xalan.extensions" select="'1'"/>
  <xsl:param name="tablecolumns.extension" select="'0'"/>
  <xsl:param name="fop.extensions" select="'0'"/>

Yesterday I send Jeff Beal my resulting FO File,
drop me a note what you want/need to see.

Since I can duplicate it, it can't be a miracle, can't it?
