I know this isn't the answer you were hoping for, but I had nothing but
problems with Steve Cheng's docbook2man program. We've had much better
luck with XML. Martijn van Beers and Tim Waugh have produced an excellent
tool set in db2man, and it's available as part of the 1.53.0 DocBook XSL
stylesheets.
