I'm not sure if it is a mistake.  I passed the question
along to Norm.  The XSL HTML stylesheets
have printed the title with the graphic since the beginning,
when 'graphical.admonitions' is turned on.  It seems
the HTML and FO stylesheets should have similar behavior
in this regard.  I think an parameter to turn off the title
is a reasonable request, though.
