Works for me.  When I process your example with:

xsltproc
1.58.1 fo stylesheet
fop-0.20.4
fop.extensions=1
ulink.show=0

I get a monofont italic version of
"http://jdbforms.sourceforge.net"; in my PDF file, and it is
a working hot link to the site.

What exactly doesn't appear in your output?
