I think you pasted the wrong command line into your
mail. This command uses the HTML chunking stylesheet,
and won't produce FO output.
You are correct. Sorry about that. I did in fact use the
nswalsh/fo/docbook.xsl stylesheet when i was generating for pdf output.
PassiveTeX does have problems formatting tables properly.

You might be better off using FOP instead.
