Bob, thanks for your suggestions.  

I have done some work on it, trying different things, and have determined
that the problem had something to do with xsltproc's handling of XInclude.
 (I think I neglected to mention that I use XInclude to bring in each
chapter individually)

I wrote a script to pre-interpret the XIncludes, and now the book builds
fine - chapter numbers are all correct.

A possibly helpful aside to any xsltproc developers reading:  I discovered
that the chapter numbers in the actual chapter files were correct, but the
TOC was wrong.
