Theortically you should be able to set a pgwide="1" attribute on a table or
figure.
Unfortunately, it doesn't work in the current DocBook XSL stylesheets with
XSL-FO 1.0. For column spans to work, the fo:block with the span has to be
an immediate child of the fo:flow. But the DocBook XSL stylesheet
architecture creates a lot of nested blocks, so a table or figure block
would not meet that condition. Trying to unwind the nested blocks for one
table would be pretty tricky. My understanding is that such spans will be
easier in XSL-FO 2.0.
