 The superbe tutorial at http://www.sagehill.net/docbookxsl/ has a great
deal about print output options.

I use this daily, and perhaps it may be of use.

Hello Avi MIller,

I have an example project you might be interested in. You
can download it from:

You must follow the DocBook link on the left (old-style
homepage that still uses frames).

I should tell you early, this project is far from perfect.
So, it is not offered as a sterling example of how to
correctly format DocBook XML, but rather I offer it in the
possibility that it can help beginners. The document uses
SVG graphics and special fonts, which complicates things
with FOP.

Please feed back to me any corrections.
