You mean, if you want some random word in the middle of a paragraph to
be cursive fat?

The answer is, you can't. That's not what structural markup is all about.
Rather, you should author descriptively, not presentationally:
<para>This is just some <emphasis>make these words fat cursive</emphasis>
random blather</para>
Then change the stylesheet to make all occurances of emphasis be
formatted as fat cursive, if you wish.
