The book itself was published using a special DSSSL to MIF stylesheet
and rendered with Frame, so there are probably typographic features
that will be difficult to achieve with the RTF or TeX backends.
For headers and footers, though, it should just be a matter of selecting
the right elements. I suggest you post your question on the docbook-apps
mailing list if you're looking for more specific aid.
