Well, this is technically a stylesheet or presentation issue, but
practically speaking I don't know of any formatter that can do
it. (Er, the Arbortext formatter might be able to...)

Can you describe the context in which you want to do this?
I mean you could just use paragraphs:
<para role="rationale">...</para>
And conditionally print those with a sytlesheet.
