Problem with table that spans multiple pages

Hello,
I'm having the following problems with a table that spans multiple pages in
a pdf document:
- There is one blank page between table title and the table itself.
- The bottom of the table isn't properly formatted (lines are sticking out).
- The header of the table is missing a line on the top.
To see what I mean please have a look at

Using:
- Docbook XML 4.1.2
- DocBook XSL 1.49 (default settings)
- Instant Saxon 6.5
- XEP 2.7 evaluation
Could this be a problem with the XSL stylesheets? Or am I doing something
wrong?
Anyone else *not* having these problems? If so, could you please tell me
about your setup?
