I can't reproduce this bug, but I do notice several odd things about your file:

| <?xml version="1.0"?>
It claims to be XML.

| <!DOCTYPE book PUBLIC "-//OASIS//DTD DocBook V3.1//EN"[]>
Then points to an SGML DTD. You probably want ...DocBook XML V4.1.2... here.
