There is the generic inline container <phrase>,
but there is no generic structural container.
Adding a generic structural container would make
the structural rules a bit fuzzy, wouldn't it?

Exactly what could be contained in what?

Well, right now I'm looking at artifacts ('templates') used when documenting software architecture, or high level design. And yes, may be something like 'role' would be good enough, but as you state, it's not even preserved in all cases.

And then, I was hoping to get some stylesheet support for
automatically generating indexes for specific kinds of these
elements.

I guess I'm not clear why role with suitable customization
doesn't meet this need.

yes, it appears customization is the way to go, though I can't find much documentation about how the current stylesheet layout supports this. Any suggestions ?
