This moves the annotation out of line and allows more than one element
to point to it. (annotations is an IDREFS, though then folks will
wonder if xlink:href works there...)

And what about allowing annotation almost everywhere (like indexterm) with the following semantic:


- if annotation have xml:id then it could be referenced by IDREFS attribute from other elements
- if annotation doesn't have xml:id then it belongs to its parent


This will allow users to select style of annotating that they prefer.

Thoughts?
