[PATCH] Cleanup i386v4.c


This patch cleans up i386v4.c.  I reformatted some comments, aaded
some doco, unified #ifdef usage, removed unnecessary prototypes etc.

I also slipped in a change that does change the logic a bit.  The
number of general-purpose registers is now fixed at 16 by the use of
the constant I386_NUM_GREGS.  This seems to be correct for all
supported SVR4 configurations.  The presence of the FPU registers is
now determined by looking at FP0_REGNUM, as we do in various other
bits of code in GDB.  I also removed FPREGSET_FSAVE_OFFSET since it
wasn's used anywhere else in GDB.

Tested on i386-pc-solaris2.6.
