[cplus] Add initial C++ name parser


FYI.  I've merged the mainline to drow-cplus-branch, gotten it into roughly
working state, and added the below patch.  It doesn't work as is:

  - My last grammar rewrite I never finished updating the actions to
    match.
  - It still does not handle declarators correctly; as a consequence
    array and pointer-to-function types are a bit broken.
  - See all the FIXMEs.

But I wanted to get it into CVS so I would have a revision history :)  Next
will be to fix up those things, or to hook it into the C++ printing
machinery.
