[commit] Get rid of duplicated fetch_link_map_offsets() code


The attached patch gets rid of all the (identical) copies of the code
in svr4_ilp32_fetch_link_map_offsets() and
svr4_lp64_fetch_link_map_offsets().  It's fairly mechanical, and since
I carefully checked the numbers, should make absolutely no difference.

This patch makes an upcoming solib-svr4.c patch necessary to fix PR
2063, much simpler.
