IMHO what is badly needed is some way to integrate documentation with
the source code, such as for instance SCWM or Emacs is doing it.

Such an extractable manual would be a poor introduction/tutorial but a
usable reference manual and would provide a quicker a better road to
better documentation, since it would be easy to hack up little info on
a quick run through of a file, add more substantial info when actually
working on some features and to keep it up to date when changing
something since the documentation is close to the point of change and
can be made/changed/committed in the same run.

It has been discussed in the past, but I do not think a decision was
ever really made. Putting a proposal together would be extremely
usefull, right here and now.
