Revised MIPS NPTL support


Here are the MIPS-only bits of NPTL support.  This depends on the
patch to fix MIPS builds on HEAD; and the typo fix for
elf/tls-macros.h; and (for LinuxThreads) the INCLUDE_TLS_PADDING
patch I just posted.
