well strange as it may seem, the parser error message
is telling you what's wrong:-)
<xsl:stylesheet xmlns:xsl="http://www.w3.org/1999/XSL/Transform" >
xsl:stylesheet has a required version attribute, so it should be
<xsl:stylesheet version="1.0"
xmlns:xsl="http://www.w3.org/1999/XSL/Transform" >
