I have a working example of how to do so (using an XT extension) in the 
annex of the free download of our book "Practical Transformation Using XSLT 
and XPath" available off our web site.

Essentially, one uses:

   <xt:document href="---filename---">
     <!--contents of file-->
   </xt:document>
