No decisions have yet been made on these questions. You'll have to wait for
the XSLT 2.0 drafts.

Don't know.
> Does the word monotonically (11.2.1.3) need to be there?
I would have thought it was a word that we could expect the educated reader
to understand. I don't think you'll find "double precision floating point"
in a layman's dictionary either.
