The attribute and valua-of instructions are in wrong namespace
<fo:table-cell>
<xsl:if test='@colspan'>
<xsl:attribute name="number-columns-spanned">
<xsl:value-of select="@colspan" />
</xsl:attribute>
</xsl:if>
</fo:table-cell>
