They're arbitrary, just like named anchors in HTML. From a DocBook document I maintain:

...in the HTML version.

If you want to play with some working DocBook, download MySQL++ from http://tangentsoft.net/mysql++/ and go into the doc/userman directory. The README file in there should be enlightening. Type 'make' in that directory to create the HTML version of the manual.
